Cancellation Policy

Monthly visits include consultations, diagnosis, and prescription call ins. Labs, injections, x-rays, and/or any additional services will be charged extra as outlined in additional service fees menu. Credit card processing may be subject to a 3.5% surcharge. Any returned checks will be charged a $50 NSF fee. Appointment cancelations must be made 24 hours prior and will incur a $50 cancelation fee if not made within the allotted time. No shows will be charged a $150 rescheduling fee. Last minute unforeseen circumstances will be taken into consideration.
